2020—2024年深圳市职业健康检查疑似职业病及禁忌证检出情况分析

Findings on suspected occupational diseases and contraindications in occupational health examinations in Shenzhen from 2020 to 2024

    目的 分析深圳市某职业病防治院进行职业健康检查的作业人员疑似职业病及禁忌证检出特征,为优化职业病防治措施提供科学依据。
    方法 收集2020—2024年在深圳市某职业病防治院进行职业健康检查的劳动者的体检资料,对疑似职业病、职业禁忌证及复查情况进行分析。
    结果 疑似职业病检出率0.07%;危害因素以噪声为主,疑似职业性噪声聋占比逐年增高,由2020年的47.83%上升至2024年的84.62%;其次为粉尘、苯系物及手传振动。职业禁忌证检出率1.44%,噪声作业禁忌证占比最高(30.38%~57.03%),但逐年下降;放射性危害因素、职业机动车驾驶作业检出比例有上升趋势。上岗前和在岗期间应复查率较高(9.50%~12.22%),但整体应复查未复查率居高不下(32.14%~59.32%),严重影响目标疾病判定。
    结论 深圳市职业病防控需重点关注噪声作业,建议构建“政府-企业-劳动者-医疗机构”协同机制,全面提升深圳市职业健康保障水平。

     

    Abstract:
    Objective To analyze the characteristics of (suspected) occupational diseases and contraindications identified through occupational health examinations at a certain occupational disease prevention and treatment hospital in Shenzhen, providing scientific evidence for optimizing occupational disease prevention measures.
    Methods The occupational health examination data at a certain occupational disease prevention and treatment hospital in Shenzhen conducted between 2020 and 2024 were analyzed; the suspected occupational diseases, occupational contraindications, and the work on re-examination were specifically reported.
    Results The detection rate of suspected occupational diseases was 0.07%. The predominant hazard factor was noise, and the proportion of suspected occupational noise-induced deafness increased annually, rising from 47.83% in 2020 to 84.62% in 2024. Other significant hazards were dust, benzene series compounds, and hand-transmitted vibration. The detection rate of occupational contraindicatio ns was 1.44%. The noise-related contraindications accounted for the highest proportion (30.38% to 57.03%) but showed a decreasing trend year by year. In contrast, contraindications related to radiation hazards and professional motor vehicle driving showed an increased trend. The required re-examination rate for pre-employment and on-duty assessments was relatively high (9.50% to 12.22%), but the rate of non-compliance with re-examination remained persistently high (32.14% to 59.32%), which may significantly hinder the diagnosis of suspected occupational diseases and contraindications.
    Conclusions The prevention and control of occupational disease in Shenzhen should prioritize noise exposure. It is recommended to establish a collaborative mechanism involving "government-enterprise-workers-medical institutions" to comprehensively improve the level of occupational health protection in Shenzhen.
