吸入甲醇致中毒性心脏病1例报告

A case report of toxic cardiomyopathy caused by methanol inhalation

  • 摘要: 近年来烟花制造企业急性甲醇中毒频发,但致严重心脏损害者罕见。本文报告了1例吸入高浓度甲醇致中毒性心脏病患者的诊疗经过。分析发现,某烟花制造企业造粒岗位因使用含甲醇黏胶剂且防护不当导致工人中毒。患者因右冠状动脉严重痉挛闭塞引发急性下壁心梗,其发病机制可能涉及交感神经兴奋及血管舒缩因子失衡。案例诊疗过程提示:尽早足量血液净化疗效显著、预后良好,但重度中毒者常因意识障碍掩盖胸痛主诉,临床上应加强心电图与心肌酶谱动态监测,以防漏诊心肌缺血。本研究为不明原因心梗病因分析及甲醇中毒致心肌损害的早期诊治提供参考,有助于改善患者预后。
