补肺活血胶囊联合经筋温针灸、光谱治疗血瘀型壹期矽肺疗效的初步观察

Therapeutic effect of Bufei Huoxue capsule combined with Tendon warm acupuncture and spectral therapy in the treatment of stage Ⅰ silicosis with blood stasis

      目的  探讨补肺活血胶囊联合经筋温针灸、光谱治疗血瘀型壹期矽肺的临床疗效，以期为中医药防治血瘀型壹期矽肺提供新方向。
      方法  选取血瘀型壹期矽肺患者104例，随机分为综合组和传统组。传统组予以汉防己甲素片治疗，综合组予以补肺活血胶囊温针灸肺俞次、膈俞次及光谱治疗40 d。两组均给予相同氧疗、饮食调理和基本治疗等常规治疗。两组均治疗7个月，治疗结束后评定其肺活量、第一秒用力呼气量占用力肺活量百分率（FEV1/FVC）、呼吸症状、活动能力、疾病对日常生活的影响、焦虑阳性率、安全性观察等。
      结果  圣乔治呼吸问卷调查显示，综合组治疗后呼吸症状、活动能力、在减少疾病对日常生活的影响方面优于传统组（P < 0.01）。二组治疗后焦虑阳性率差异无统计学意义（P > 0.05）。治疗后综合组肺活量、FEV1/FVC均高于传统组（P < 0.05）；安全性观察两组均未见明显异常。
      结论  补肺活血胶囊联合经筋温针灸、光谱治疗血瘀型壹期矽肺有一定的优势，可以活血化瘀、消炎止痛、改善患者症状，发挥中西医结合治疗对矽肺的优势特色，值得在临床上进一步研究使用。

     

    Abstract:
      Objective  To explore the clinical efficacy of Bufei Huoxue capsule combined with Tendon warm acupuncture and spectral instrument in the treatment of stage Ⅰ silicosis with blood stasis, to provide new guidance for treatment of traditional Chinese medicine in silicosis with blood stasis.
      Methods  Totally 104 patients diagnosed as Stage I silicosis with blood stasis were randomized into traditional treatment group and comprehensive treatment group. Except for the oxygen therapy, diet regulation and basic treatment in both groups, the patients in the traditional group were treated with tetrandrine tablets and the patients in comprehensive group were treated with Bufei Huoxue capsule combined with Tendon warm acupuncture and spectral therapy for 40 days. After treatment for 7 months, the patients in both groups were examined and the indicators were compared.
      Results  The St. George's respiratory questionnaire showed that the patients in comprehensive group had less respiratory symptoms, better activity ability and less effects of disease on daily life than the patients in traditional group(P < 0.01). There was no significant difference in the positive rate of anxiety between the two groups after treatmen(t P > 0.05). After treatment, the vital capacity and one second rate of patients in the comprehensive group were higher than those of the patients in traditional group (P < 0.05);there was no obvious abnormality of safety in both groups.
      Conclusions  Bufei Huoxue capsule combined with Tendon warm acupuncture and spectral therapy had advantages in the treatment of stage I silicosis with blood stasis, which could improve the patients' respiratory muscle fatigue, lung expansion ability, respiratory symptoms, activity ability and the influence of disease on daily life. It is worthy of further study and application in the clinic.
