广州市核医学工作人员甲状腺131I调查结果分析

Investigation on Thyroid 131I levels of nuclear medicine staff in Guangzhou

    目的 了解广州市核医学工作人员甲状腺中131I的活度水平,估算核医学工作人员所受的内照射剂量,评估核医学工作人员的内照射风险。
    方法 2023年9—11月,采用方便抽样的方法,选取广州市4家开展核医学诊疗医院的50名核医学科工作人员为研究对象,使用便携式RT31EPT15型甲状腺测量仪直接测量核医学工作人员甲状腺中131I的活度水平,估算被测人员131I的摄入量,推导其待积年有效剂量。
    结果 50名核医学工作人员中,甲状腺检出131I核素的人员有7名,检出率为14%。不同性别、岗位人员检出率差异均无统计学意义(P > 0.05)。检出甲状腺131I的7人中,131I检出活度范围值为60.8~143.6 Bq,均值84.1 Bq,中位数83.7 Bq,医师、技师、护士的年有效剂量水平分别为0.29 mSv/年、0.38 mSv/年和0.54 mSv/年。
    结论 广州市4家医院50名核医学工作人员的内照射年有效剂量值均在标准限值内,但内照射风险仍不容忽视。应在核医学工作人员间开展内照射剂量常规监测,切实保障核医学工作人员的职业健康。

     

    Abstract:
    Objective To determine the activity levels of 131I in the thyroids of nuclear medicine staff in Guangzhou, estimate the internal radiation dose received by these staff, and assess their internal radiation risk.
    Methods From September to November 2023, a convenience sampling method was used to select 50 nuclear medicine staff from four hospitals in Guangzhou that conduct nuclear medicine diagnostics and treatments. A portable RT31EPT15 thyroid measurement device was used to directly measure the activity levels of 131I in the thyroids of the staff. The intake of 131I was estimated, and the annual committed effective dose was derived.
    Results Among these 50 nuclear medicine staff, 7 individuals were found to have detectable levels of 131I in their thyroids, with a detection rate of 14%. There were no statistically significant differences in detection rates among different genders and job positions(P > 0.05). Among 7 individuals with detectable 131I, the activity levels ranged from 60.8 to 143.6 Bq, with a mean of 84.1 Bq and a median of 83.7 Bq. The annual effective dose levels for physicians, technicians, and nurses were 0.29 mSv/year, 0.38 mSv/year, and 0.54 mSv/year, respectively.
    Conclusions The annual effective dose values of internal radiation for these 50 nuclear medicine staff in four hospitals in Guangzhou were within the standard limits. However, the risk of internal radiation exposure should not be overlooked. Routine monitoring of internal radiation doses among nuclear medicine staff should be conducted to ensure their occupational health.
